Years ago someone came up with On Beyond the Peter Principle which I think is an accurate reflection of many workplaces. When it comes to that next promotion or opportunity, you can't afford to let the good employees go so you choose someone who is not totally useless but has already reached his or her level of incompetence and won't be missed. Hence the dysfunction of much of middle and upper management.
